How a Clinic Becomes A Partner
The BeWise Program is always looking for providers who will agree to provide follow-up care and become a primary care provider for women who come through the program. To become a partner with the program simply fill out and fax or email the completed form to [email protected] or (fax) 801-536-0172.
Refer a Client
If you have a patient you think is eligible for the program, referring them is easy. Simply have them call
1-800-717-1811 to see if they qualify and we will link them to the closest contracting location for services.
